Job Description:
The Ambulatory Care Pharmacy Intern will assist in providing direct patient care services within the anticoagulation clinic under the supervision of a clinical pharmacist. This position offers pharmacy students the opportunity to gain hands-on experience in managing anticoagulation therapy, including patient education, monitoring, and dose adjustments. The intern will develop clinical skills related to anticoagulation management and enhance their knowledge of evidence-based guidelines.
Minimum Qualifications:
• Must be actively enrolled in an accredited Doctor of Pharmacy (PharmD) program.
• Must hold a current and unrestricted Ohio Pharmacy Intern License within 90 days of hire and maintain thereafter.
• Strong interest in ambulatory care and anticoagulation management.
• Excellent communication and patient counseling skills.
• Ability to work independently and collaboratively in a team environment
• Basic knowledge of anticoagulation medications, monitoring parameters, and guidelines (preferred)
• Experience with electronic health records and point-of-care testings (preferred).
• Demonstrates effective and culturally appropriate communications.
Physical Requirements:
1. Visual acuity of 20/20 or corrected to 20/20 including color vision when applicable.
2. Ability to hear (with or without aides).
3. Ability to orally communicate.
4. Good manual dexterity.
5. Ability to push, pull, roll, and transfer/lift 50 pounds occasionally.
6. Ability to stand, walk constantly.
7. Ability to bend, reach, stoop, and kneel occasionally.
Work Schedule and Requirements
• Flexible based on clinic hours.
• Must be able to commit to assigned hours per week based upon clinic needs.
