The Clinical Referral Liaison-Transplant position provides support for the referrals and Transplant Administration after the patient has been financially approved and moving forward to the multidisciplinary evaluation appointment. The coordination of clinical information from referral to evaluation is needed to prepare the team for the complex medical history that can be encountered with Transplant evaluation patients. This will require the ability to navigate/research patient’s medical, social, medication and prior evaluations/work up history at previous hospitals/transplant centers. This position will work collaboratively with Transplant Assistants, Transplant Coordinators, Financial Coordinators and providers within the Transplant team.
